Levy J. Collins, IV, 31, a native of Golden Meadow and resident of Galliano passed away on Friday, May 15, 2015.
Visitation will be held at Samart-Mothe Funeral Home in Cut Off on Wednesday, May 20, 2015 from 9:00 a.m. until funeral time. Funeral Services will be held at the funeral home at 11:00 a.m. with burial to follow in Our Lady of Prompt Succor Cemetery in Golden Meadow.
He is survived by his wife, Brandi L. Collins; father, Levy J. Collins, III; mother, Susan M. Collins; son, Levy J. Collins, V; daughters, Zoe Collins, Sadie Claire Collins; step daughters, Emmie Tolley, Macey Tolley.
He was preceded in death by his brother, Robert Collins.
